This happened a week or so ago and you couldn’t recreate the error at that time so I made a screenshot this time.
The log did not clear the call signs after I entered NZ5E and NZ5W, so I made sure to watch on the next one which was N5KWD. After I submitted it to the log the call sign remained AND it is showing as a Dupe.
And I just noticed that NZ5E and NZ5W are showing different states although they are in the same park, US-2790.
Got myself a trifecta. 
We have also had this happen a few times.

Just another example. I thought I had seen it a couple more times today so I specifically watched for it on this last contact. I submitted the contact, it logged it, and it put that contact’s call sign back into the field instead of clearing the field (and then obviously showed it as a dupe).
That happened to me as well. If you look at their QRZ pages, one is from OK, the other from TX. They were operating the same park together, but I think only one was listed is the OP in the park. So, WRL pulled from one of their QTH’s and entered that.

I did a Shift-Refresh and then pulled up the April 18th log; NZ5E and NZ5W are still showing different states and mileages although they were both in the same park at the same time, and the same park number is recorded in both log entries.
@W7YBY Hey Brian,
The fix was released so this doesn’t happen again but the data in your logbook wasn’t updated. I’ve manually updated it and its now showing correctly for both those contacts.
